Do you mean a restart of Otter or the system?

Otter has 3 content blocking options:
1. per tab ("Blocked Elements" button on the bottom right corner), which has higher priority than:
2. per domain (right mouse button -> Website Preferences...), which has higher priority than:
3. global (menu Tools -> Content Blocking...).

So globally you have to set content blocking in menu Tools -> "Content Blocking...", but it can be replaced by filters in website preferences, which can be replaced by filters in "Blocked Elements" button.
So, for example, if you set filters in "Blocked Elements" button on the tab with some website, you have to open this website on different tab, in order to filters from global content blocking will be enabled on this website.

Yes, quite complicated...
